Nurse, Nursing Assistant or Medical Assistant: Who are they and how are they different? (Part 2 of a two-part blog post)

Nurse, Nursing Assistant, or Medical Assistant?

In yesterday’s post I explained what qualifies individuals to assume the title of nurse in its two major variants: Licensed practical nurse (LPN) and registered nurse (RN). Today I look at two other healthcare professionals who are sometimes mistaken for nurses. Nursing assistants (NA) and medical assistants (MA) are valuable members of the healthcare team who also provide direct care to patients in various healthcare settings. These roles are often confused with nurses.

Nursing assistants complete a state approved program, usually 75-96 hours in length and pass an exam to be certified and placed on a state registry. The role of a nursing assistant is to provide basic care related to hygiene, nutrition and mobility needs. The nursing assistant is directly supervised by a registered nurse in a hospital, or home or long-term care setting.

Medical assistants complete a course of study in an accredited program with an end
degree of an associate of applied science, and are recommended to take a certification exam. The role of a medical assistant is to serve the needs of physician employers in clinic settings. The medical assistant is supervised by an advanced practice registered nurse and/or physician and may administer medications, such as immunizations, perform blood draws, assist in clinic procedures, and perform administrative duties.

Clearly, it takes a wide range of health professionals to deliver patient care when and
where it’s needed. From nurses and nursing assistants to medical assistants, each is qualified to perform a wide range of care related responsibilities. It’s the person’s education and state regulations that determine what those responsibilities are.

MSB senior nursing students Molly MacLean and Katie Theisen were invited to write about nursing salons for an article in the upcoming August 2012 issue of Creative Nursing: A Journal of Values, Issues, Experience & Collaboration. Molly and Katie attended their first nursing salon as part of a class assignment, but have been attending regularly for the past year. At one of the salons, the journal editor was impressed with them and invited them to submit an article on salons from a nursing student perspective.

Here’s an excerpt…

At a Nursing Salon, the format is always the same but the conversation is always different. Dinner is served. Then everyone introduces themselves and answers the question, “What is on your mind about nursing tonight?”

Once everyone has responded, dessert is served and the group can begin discussing a few of the topics brought up. Sometimes the topics are personal; sometimes the topics are large and broad, but the conversation always stays in the room. The sense of confidentiality at the Nursing Salons makes me feel comfortable to say what I really feel, to be myself and to feel the authenticity of the whole group. The conversations are honest, exciting, motivating, and experiential. No matter the topic, the time spent at a Nursing Salon is always valuable to me. Nursing Salons have given me clarity about school, life, priorities, and my future.

On May 14, 2012, former Governor Tim Pawlenty spoke at the
University of Minnesota about “Restoring America’s Future.” Although he made interesting points about teaching credentials and rigor in education programs, I do take issue with a comment he made about teacher compensation. I recognize that I have a bias with over ten years of coaching and five years of involvement with personal trainer education, however, allow me to explain.

Pawlenty incredulously and disrespectfully questioned the validity of a high school physics teacher making the same amount of money as a high school “gym teacher.” He states, “Really? We are going to pay a physics teacher the same as the gym teacher?” He goes on to say something about physics teachers teaching “mission critical” skills to students, but that he isn’t discounting the importance of a gym teacher.

Really?

Let’s clarify a few things for T-Paw. In fact, let’s start with the notion of “mission critical.” Obesity, diabetes and heart disease are three epidemics that have the power to ultimately destroy our health care system. Additionally, this generation of children could be the first that do not outlive their parents. I’d call that mission critical.

All of these epidemics can be addressed with lessons learned from physical education teachers, i.e., MOVEMENT! I’m not discounting physics teachers at all. However, the students will perform better in physics if there is a physical education teacher at the school. Studies have repeatedly shown than movement is critical to cognitive development. It has to do with BDNF; a chemical that is released during physical activity. John Ratey, author of Spark, calls it “Miracle Grow for the brain.”

Ratey decidedly proved in his book Spark that students engaging in physical activity not only were healthier, but also scored higher on standardized tests. Honestly, what is more
important than teaching kids about their own health and well-being? Clearly enough of it isn’t being done when over 30% of youth are overweight or obese.

I’d like to direct Tim to the many libraries available at the University of Minnesota that house multiple research articles indicating that vigorous exercise leads to improved academic achievement. Or check out the Star Tribune’s recent article about Meadowview Elementary School’s successful integration of physical activity in the classroom.

I have to wholeheartedly disagree with the former governor. To me, physical education teachers are worth their weight in gold.

Concern for our planet with regards to our increase in population coupled with declining natural resources is on the rise. Architecture once again has taken the forefront in rethinking urban and architectural design with sustainability in mind. The Architectural Drafting and Design program at Globe University is no different! Green building technology and LEED (Leadership in Energy and Environmental Design) concepts are incorporated into the curriculum.

This is not a new topic however. With a myriad of books and experts speaking on the topic, one individual stands out to me who has an incredible grasp of our future and I highly recommend reading his work. His name is Paolo Soleri! If you are interested in reading about his philosophy I highly recommend The Urban Ideal and the Mind Garden, two exceptional books. The image is a cast metal bell I purchased during a visit to his studio and workshop. My next trip to Arizona will include visiting Arcosanti and possibly attending a week-long workshop Arcology, the process of architecture and ecology.

The proceeds from the sale of windbells through the Cosanti Foundation fund his research community Arcosanti in the desert outside of Scottsdale Arizona. Stop back for future discussions on architecture and sustainability!

Though most of the money would eventually flow to healthcare providers, insurers would keep an average of $22 billion per year for profit and administrative costs, the study said.

The Supreme Court is expected to issue its decision on healthcare reform before the end of June. If upheld, the law is projected to expand insurance coverage to 32 million people by 2016.

Meanwhile, the United States falls behind other industrialized nations on measures of health care quality, in spite of all this spending. READ MORE:

Field trips rock or in this case splinter! The Woodbury Architectural Drafting and Design students are exploring residential construction material and processes through an interactive web-based software, classroom lecture and a recent field trip.

Villaume Industries is a Twin Cities leader for well over 100 years and has once again graciously opened their doors and allowed us the opportunity to tour the manufacturing facility. The tour started in the engineering office where students experienced the software used to calculate components, code and load requirements for custom roof and floor truss projects. Upon the completion of the shop drawings, the tour continued to the factory floor where technology is used to reduce waste and increase the speed and accuracy in which trusses are manufactured. It was exciting to hear that production has increased significantly over the past year which is a sign of an upturn in the residential construction economy.

Roof and floor trusses have significant advantages over roof rafters and floor joist. Trusses are more cost-effective, quicker to install, span greater distances are can support greater loads. Environmentally speaking, trusses have additional advantages to large dimensional lumber as well.

The image illustrates the anatomy, physics and basic mathematical concepts behind a Pratt Roof Truss.

Architectural student throughout the 6 quarter program continually increase their understanding of building materials and process which are essential for accurate drawings and efficient construction. More tours are being schedule and if you’re interested in joining us, please contact John Hartman, Globe University, Woodbury campus room 228.

The students also went over to see in-custody arraignments – first appearances for people facing criminal charges. They also participated in a tour of the Hennepin County Law Library guided by a reference librarian and met with the staff at the County Assessor’s Office.

You may be following the news about JPMorgan’s $2 billion trading loss that is currently rattling financial markets. Would you believe that the investment that caused this loss, called a credit default swap, is the exact type of investment that caused the financial crisis?

A credit default swap is essentially an insurance policy on a bond or group of bonds. If you own a bond, one of the risks is that the issuer of the bond will stop making payments, which is called a default. You can protect yourself against this risk by purchasing a credit default swap (CDS). Just as you do with car insurance or homeowner’s insurance, when you buy a CDS, you pay a small premium. In return, you are compensated when you experience a loss. A credit default swap is an important and perfectly legitimate method of protecting yourself from loss on an asset that you own.

On the other hand, a credit default swap can also be used as a pure bet. For example, you can purchase a CDS on an asset that you don’t own. As an analogy, this would be like you purchasing car insurance on a driver you don’t know in a different city. You make premium payments and hope that this unknown driver gets into a wreck so that you are paid off on the policy.

As another example, you could sell a CDS, which means that you collect insurance premiums from people, but have to pay them off when they experience a loss. This is what insurance companies do. It works when you have good information about the risks (for example, the average number of car accidents), and you keep a pool of funds large enough to make payoffs when people get into accidents. However, in the financial crisis, investment banks were selling enormous numbers of insurance policies on subprime mortgage bonds, without nearly enough funds to make payoffs when these bonds went bad. Imagine telling someone about to jump off a six-story building, “Give me $1 for an insurance policy, and I’ll pay you $1,000 if you break your leg.” Except do this for a billion people all about to jump off a building.

The current situation with JPMorgan shows that we have not done enough to prevent future crises. The bank has repeated the same mistakes that led to the bailouts of 2008-2009, as well as the bailout of LTCM in 1998 under astonishingly similar circumstances, the savings and loan crisis of the 1980’s, and a string of corporate bailouts going back to at least 1971. What do you think we should do to stop this cycle of companies becoming “too big to fail,” having virtually no restrictions on their actions, and then receiving a taxpayer-funded bailout?

The challenge is how to instruct health care management students on the future needs and trends in the health care environment. With the regulatory field rapidly changing, and the pace of innovation, there is a challenge to keep health care management programs current and relevant.

Militello and Sheppeck (2012) completed a survey of 70 health care organizations to determine what the organizations see as challenges for the Affordable Care Act passed in 2012. The researchers are both University of St. Thomas professors teaching in the health care management area of the Opus College of Business. The research was prompted by the integration of health care reform legislation (e.g. the Patient Protection and Affordable Care Act) and educational reform legislation (e.g. Health Care Education Reconciliation Act) into the overarching law collectively referred to as the Affordable Care Act (ACA). This marriage of two different pieces of legislation is particularly important for instructors in current health care management programs.

While the legislation is clear on the mandate for health insurance coverage for citizens, it provides no direction on how health care organizations should meet the potential changing needs of health care. The researchers sampled over 500 organizations with a completed response from 70 organizations scattered across the country.

The survey questions centered on how organizations were planning, or orienting planning toward the future. The underlying belief is that strategic planning is a necessity to keep any organization viable. The researchers were interested in determining what type of strategies currently existed and what types of strategies may be needed. The idea was to determine gaps in current knowledge areas that may help health care management programs better prepare graduates to meet.

The data set findings are interesting. The data separated out into three unplanned clusters. Forty percent of the sample showed a defined strategic focus. These organizations were seeking out ways to differentiate their business plans from the competitive market. This group was looking at developing unique forms of patient care. These organizations were actively recruiting new and innovative talent.

Thirty four percent of the sample was defined to be in the second cluster. These organizations appeared to be at the median of all measured variables. It seemed that these organizations represented groups that were cautious. These organizations were not doing anything novel and seemed worried about the volatility of the health care industry.

The third cluster made up the remaining 14%. These organizations seemed unable to move in any direction. Either management or the underlying governance structure was either unable or unwilling to consider any change or new prospects in the way health care is delivered.

The researchers seemed discouraged with the overall findings. Militello and Sheppeck (2012) noted the recent legislation is looking at a significant consumer focus and consumer involvement in creating accountable care organizations. Further, the legislation leaves open the door for innovation and novel approaches to treat a population through a preventive focus versus the current reactive focus. It appears that many health care providers do not have a strong understanding of market focus or consumer need.

This research is interesting and germane to Globe University and health care management programs in general. Since Globe is training today’s and tomorrow’s health care leaders, we need to consider how we integrate the idea of innovation, market driven need, a change in health care delivery model, and the need to take business risks.

By Brian Craig, Paralegal Program Chair

Starting in Early Fall Quarter 2012 in July 2012, paralegal students with the Globe Education Network will receive a Westlaw password that they will be able to use during the entire time they are enrolled as a student in the paralegal program.

Currently, students taking certain courses, such as LA155 Legal Research I, pay a lab fee and receive a Westlaw password from their instructor, but have to re-register a new Westlaw password each quarter when they take a course with a Westlaw password. This process is somewhat burdensome since new passwords have to be issued each new quarter.

Under the new system that will take effect Early Fall Quarter 2012, each paralegal student will receive their own Westlaw password that they will be able to use during the time the student is enrolled as a student in the paralegal program. Because Westlaw passwords can only be used for educational use, students will not be able to use the Westlaw password during the internship at the end of the student’s program. Therefore, the student’s password will be deactivated when the student starts the internship at the end of the program.

Students will receive the Westlaw password/activation key from the paralegal program chair from their home campus. Students whose home campus is online (Globe University Online, Minnesota School of Business Online, Broadview University Online) will receive their Westlaw password and registration key from Brian Craig, paralegal program chair for the online division, via email, on or before July 29, 2012. Students taking online courses but with a home campus other than online should contact the paralegal program at their home campus.

Students taking the following courses will pay a lab fee of $40 per course for Westlaw usage:

LA155 Legal Research I

LA215 Law Office Procedures and Technology

LA235 Contracts

LA240 Legal Writing I

LA265 Family Law

LA300 Law Office Management

LA323 Bankruptcy

LA410 Legal Research II

LA425 Legal Writing II

LA450 Field Preparation

If a student already has a West OnePass account, the student will simply need to click on the link to activate the registration key that they will receive in an email at the beginning of Early Fall Quarter 2012 in July 2012. If a student has not yet created a West OnePass account, the student will need to create a new West OnePass account. Students should use their school email address when creating the West OnePass account and should create a username and password that is easy to remember.

More information on how active a Westlaw registration key will be forthcoming in July 2012.

Paralegal students will have access to both classic Westlaw at http://www.westlaw.com and WestlawNext at www.next.westlaw.com. Students are encouraged to use both classic Westlaw and WestlawNext but students should spend more time on developing legal research skills using WestlawNext since eventually, classic Westlaw.com will be phased out. Students will continue to have access to LexisNexis Academic available through the Online Library available via the “Library” link in all online courses in CampusConnect.

With WestlawNext, students will now also have access to jury verdicts and settlements. Students will continue to have access to other databases on Westlaw, including cases, statutes, regulations, secondary sources, forms, and news. Because of privacy laws, students do not have access to public records databases on Westlaw or WestlawNext.

Westlaw passwords should only be used for educational purposes. Examples of educational purposes include working on class assignments, completing Westlaw e-learning tutorials, and learning how to research and navigate on Westlaw. Students will not be able to use Westlaw passwords for employers during internships. If a student is currently working or interning for a law firm or other organization, the Westlaw student password should NOT be used for work-related projects. Utah State Bar Opinion No. 11-03 (Nov. 15,
2011) states that it is a violation of the Rules of Professional Conduct for an
attorney to ask a law student [or a paralegal student] to undertake research
using the student’s account and in breach of the student’s contract with Lexis and/or Westlaw.

Students needing assistance can call Westlaw at 1-800-WESTLAW (1-800-937-8529) for customer service, technical, or research assistance.

We are excited about the new change in Westlaw passwords and hope that this new process will help students continue to further develop their legal research skills.
